DaVita Integrated Kidney Care (IKC) is looking for a Full-Time RN Case Manager (RN) to join us in Kissimmee/Celebration, FL region. * Must live locally*

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. Through medical record review and consultation, you will identify the medical, social, emotional, and financial needs of your patients with CKD/ESKD and implement appropriate interventions. You’ll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.

Position Details:

1. Location: Hybrid-Remote. Work from home (telehealth) and travel within an assigned geography for in-person visits (dialysis clinics and nephrology practices).

2. Schedule: Monday- Friday with flexibility to accommodate patient and nephrology partner’s availability.

3. Position type: Full-time.

4. Build empathetic relationships with patients and families.

5. Coordinate care, manage transitions, and establish treatment plans with the care team.

6. Address social determinants of health including medical, emotional, educational, community, and financial concerns.

7. Ensure effective transition of care when needed.

8. Assess patients for unreported medical conditions or changes that may lead to adverse outcomes.

Requirements:

1. Passion for care of patients with complex, chronic illnesses.

2. Flexibility and adaptability in priorities.

3. Current RN License; BSN preferred.

4. Minimum 2 years RN experience with healthcare performance coaching.

5. Current CPR or BLS certification.

6. Valid driver’s license and willingness to travel within territory.

7. Intermediate skills in MS Word, Excel, Outlook.

8. Home internet with at least 1MB upload/download speed.

Preferred Qualifications:

1. 2+ years in Case Management or Chronic Care Management.

2. 1+ years utilization management experience.

3. Spanish-speaking skills preferred in some markets.

Salary Range:
